Job Description
Case Management & Compliance:
· Maintain an average caseload of 8 foster/adoptive homes, with approximately 12 children in care.
· Conduct regular home visits and out-of-home visits to monitor child safety and well-being.
· Maintain thorough and timely documentation of case notes, home visit reports, service plans, and progress updates in accordance with state and agency standards.
· Ensure compliance with Community Connections policies, RCCL guidelines, and RBWO standards.
· Prepare and submit documentation to DFCS, the courts, and other stakeholders as needed.
· Ensure that all case management data systems are continuously updated with required and relevant case details (i.e. GA Score, extended Reach, internal files, etc.,).
Advocacy & Collaboration:
· Advocate for the child’s best interests during court hearings, IEP meetings, staffing's, and team meetings.
· Collaborate with DFCS Case Managers, foster/adoptive parents, medical and mental health professionals, educators, and legal personnel.
· Participate in the foster parent selection process and ongoing training activities.
· Equip foster families with referrals and resources to support placement stability and child success.
Crisis Intervention & On-Call Support:
· Provide weekday evening on-call coverage for assigned cases.
· Participate in rotating on-call coverage during weekends and holidays.
· Transport children, as needed, during emergencies.
· Provide support and response during critical incidents, with guidance from the Family Consultant Supervisor when necessary.
Training & Supervision Structure:
· Attend any regularly scheduled meetings as directed by the Family Consultant Supervisor.
· Participate in team meetings, agency trainings, and monthly staff meetings.
· Utilize “live learning” experiences and real case examples as part of continued training and skill development under the guidance of the Supervisor.

Qualifications:
· Bachelor’s degree (required) or Master’s degree (preferred) in Social Work, Psychology, Counseling, Sociology, Education, or a related behavioral or human services field.
· At least 1 year of case management or child welfare experience preferred.
· Reliable transportation with auto liability insurance minimum coverage of $100,000.
· Proficiency in written documentation, organization, and compliance reporting.
· Strong communication skills and a trauma-informed approach to care.

Work Environment & Schedule:
· Hybrid schedule: minimum 4 hours/week in-office; remaining hours remote, court, or community-based.
· Flex-time permitted with prior Supervisor approval.
· Mileage, parking, and child transport snacks reimbursed monthly at the federal rate.
